  • Names, phone numbers, addresses, and email addresses of the driver and all occupants of the vehicle.

 

  • Driver's license number and license plate number.

 

  • Type, color, model of the car with which you had the accident.

 

  • Insurance information (name of the insurance company and policy number).

 

  • Police report number, phone number, name and badge number of the officer.
